Welcome to our learning journey where you can explore easy-to-read infographics about different chronic conditions. After reviewing each topic, you will answer a few brief questions to share if the information helped you feel more prepared to manage your health or had no impact.   • Week 3: Finish with Joint Health and Arthritis and post-survey (or sooner if ready)
  •  Every Step: Surveys  (total of 4; pre, post, and 2 educational)
  •  Every Time: Use your same 4-digit PIN for every survey
  •  Total Process: Please finish the full study from start to finish

Privacy Policy

Protecting your privacy is our top priority. All shared data remains entirely anonymous. We do not collect names, phone numbers, home addresses, or dates of birth. The private ID you create is the only link between your Pre-Survey, Post- Survey, and Education Follow-Up Survey responses, and only you know it. Our digital systems are highly secure. Your data is used exclusively for this Boise State University quality improvement project to develop improved health resources for our neighbors. We never sell your personal information to outside parties.

Urgent Medical Care
